Registration for the 2008-2009 School Year

Pre-Kindergarten (3- and 4-Year-Olds)
Child must accompany parent

Kindergarten
Testing date will be given at time of registration

Grades 1 through 8
Testing date will be given at time of registration

Please do not come unless all information listed below is complete; otherwise we will not be able to process your application. 

* Original birth certificate
* Social Security card
* If Catholic, a copy of the child's Baptismal Certificate, which will be kept on file
* If applicable, copies of Penance, Communion and Confirmation Certificates
* Complete Immunization Record
* Proof of Residence (e.g., utility bill with your address)
* Proof of Guardianship (if applicable)
* Grades 1-8: A copy of the most recent report card and a copy of the most        
      recent standardized test scores
* There is a $200.00 non-refundable registration fee payable upon registration.
